Referral requests can be made by emailing terrimccartney@cmhaoxford.on.ca or kimbain@cmhaoxford.on.ca directly. Or you can print and complete the form below and mail it to 522 Peel Street, Woodstock, Ontario, N4S 1K3 or fax it to 519-539-8317 and it will be forwarded to the appropriate person.
